Job Title: Full-Time Statewide Local/Hybrid/Traveler Nurse Practitioner – In-Home Assessment (IHA)

Position Type: Full-Time
Nationwide Salary Range: $108,000 - $144,000
Sign-On Bonus: $2,500
Travel Stipend: $190 per day
Flexible Schedule:
· Five days a week:
o Monday to Friday, 8am to 5pm
o Average of 25 patients a week
· Four days a week:
o Monday to Thursday
o Tuesday to Friday
o Wednesday to Saturday
o Average of 7 patients a day

As a Full-Time Statewide Nurse Practitioner, you will play a pivotal role in our organization's expansion and commitment to providing exceptional care to our patients.
You will be part of a dynamic team of healthcare professionals, traveling to deliver In-Home Assessments.

Key Responsibilities: 1.
In-Home Assessments (IHA): Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management.
2.
Patient Care: Provide high-quality patient encounters, comprehensively assessing patients' health conditions, and collaborating with other healthcare professionals as needed.
3.
Travel: Travel statewide to various locations, ensuring timely and effective healthcare delivery to our patients.
4.
Documentation: Maintain accurate and timely patient records, including assessment findings, treatment plans, and progress notes.
5.
Compliance: Adhere to all relevant healthcare regulations, including Medicare and Medicaid guidelines.
6.
Patient Education: Educate patients and their families on their health conditions, treatment options, and self-care strategies.
7.
Team Collaboration: Collaborate with our interdisciplinary healthcare team, including physicians, nurses, and support staff, to provide comprehensive patient care to identify care gaps and relay concerning health findings to the health plan and PCP via escalation care.
8.
Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes and optimize healthcare delivery processes.
Qualifications Qualifications to Ensure Quality Providers for our Patients: · Master’s degree in nursing (MSN) with Nurse Practitioner (NP) certification.
· Current state NP license and national certification.
· A minimum of two years of clinical experience, preferably in primary care, family care, or geriatrics who hold an FNP, ANP, or AGNP certification.
· Strong interpersonal and communication skills.
· Ability to work independently and as part of a collaborative healthcare team.
· Proficiency in electronic health record (EHR) systems.
· Willingness to travel statewide and adapt to different healthcare settings.
· Commitment to patient-centered care and continuous learning.
